There are several problems that can affect the result of a burned CD, mainly:
1) UNLIKELY (but you never know) burner actually out of track/alignment
2) "mismatch" between burner and media (some burners will work better with a certain brand/make of Cd/Dvd), this does not mean that a certain media is not good, ut may work wonderfully on another make/brand of burner
3) burning speed, usually anything burned at the slowest possible speed is more "reliable", some burners "like" to burn at a "medium" speed, fastest available is NEVER recommended, as it often causes errors
4) wrong settings/corrupted burning software
5) old or defective burner firmware, in some cases an update is necessary/advised:

1) forget the (whatever) software you are currently using and try the Freeware IMGBURN:
http://www.imgburn.com/http://forum.imgburn.com/index.php?showtopic=5555for creating .cue file and to do the actual burning
2) forget the (whatever) software you are currently using and try the Freeware EAC:
http://www.exactaudiocopy.de/if you need a "ripper"
3) try another make/brand of media
4) ONLY if the above does not work, try upgrading the firmware
